Waiver & Release of Liability

Over 18 – I declare I am over 18 years of age (or I have provided parental consent to Intune Pilates Ltd) and acknowledge and understand that I have voluntarily chosen in the classes and activities offered by Intune Pilates Ltd.

Acknowledgement of Risk I understand that participation in Pilates Mat, Reformer and Mixed Equipment based exercise involves risks, including but not limited to: muscle strains, sprains, or other injuries; falls, slips, or accidents involving equipment; and exacerbation of pre-existing medical conditions. I acknowledge that the workouts are a recreational sports activity and may involve strenuous physical activity including, but not limited to stretches, lifts, use of props, use of reformer machines, gymnastic movements, strenuous bodyweight exercises and other strenuous activities that I am not obliged to perform, nor am I obliged to participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time during classes.

Health Declaration I confirm that I am in good physical health to participate, have disclosed any injuries, conditions, or limitations including but not limited to any injury, back, neck or joint pain, restricted movement, heart issues, asthma, or high or low blood pressure, arthritis, slipped or bulging vertebral disk, pelvic floor conditions, dizziness, bone degeneration (osteoporosis), epilepsy, hernia or chronic illness to the instructor, and will seek medical clearance if I am pregnant, postnatal, injured, or under medical treatment.

Release of Liability To the fullest extent permitted by New Zealand law, I voluntarily assume all risks associated with participation and release and indemnify Intune Pilates Ltd., its directors, employees, contractors, and instructors from any liability for injury, loss, damage, or expense, except where such liability cannot be excluded under New Zealand law (including the Consumer Guarantees Act 1993).

Equipment and Conduct I agree to follow all instructions regarding the safe use of equipment, notify the instructor if I feel pain, discomfort, or unsafe, and accept responsibility for any damage caused by misuse.

Photography & Media I agree to Intune Pilates Ltd taking photographs or videos of classes that may include me, for promotional purposes only from time to time.

Lost, damaged items I agree that Intune Pilates Ltd shall not be held liable or responsible to me for articles lost, damaged or stolen from its studio.

Acknowledgement by signing below I acknowledge I have read and understood this waiver and that I voluntarily agree to the terms and conditions stated above. This is a requirement prior to attending class.
